The medication reconciliation assignment was a way to compare the patient’s medications for its intended use. Managing a patient’s medication information is essential because it’s a way to see what the patient is taking and why they’re taking it. Often, patients are treating illnesses caused by the side effects of drugs they may not need. That’s why educating patients of side effects to medications they’re taking and teaching them nonpharmacological strategies to treat them are essential. If that doesn’t work, and it’s bothering them, they shouldn’t hesitate to reach out. If they decide to take new medications, they should reach out to their primary care provider to make sure it doesn’t interact with current medications. Polypharmacy is a current issue, which can potentially be solved by medication reconciliation.
